Terms and Conditions

• This plan is available for businesses with 5 or more employees
• As an employer, once you have chosen the option that meets the needs of your employees you have the option to change your benefit selection every 12 months thereafter
• If you should terminate your benefits with SmartChoice Benefits Inc. at any time once you have joined the plan, you will not be permitted to rejoin the plan unless evidence of insurability is provided and certain restrictions may apply on health and dental coverage
• Your rates are guaranteed for 12 months from the effective date of your SmartChoice Group Policy
• Notification will be given to you prior to any rate changes
