How To Enroll

  • Complete and sign the  Premium Authorization Agreement.
  • Complete and sign the Enrolment Form
  • Enclose both a cheque marked “VOID” and a cheque for the first month’s premium including applicable provincial sales tax.
  • Premiums will be automatically withdrawn from your business bank account each month.
  • The required forms are available under Enrolment Forms
  • Please note that your benefits and premium withdrawals will commence on the first day of the following month after we receive the original signed forms.
  • Your claims kit including a booklet and prescription drug card for will arrive during the first week of the month that your benefits are effective.
