Conversion Options

If you are no longer an owner or an employee, you may continue the following benefits at your own expense providing that you have been covered for any of these benefits listed below for at least 12 consecutive months:

• Group Life Insurance
• Group Long-Term Disability Injury & Illness
• Dental Option 1
• Health Option 1
• Vision if applicable
• Group Critical Illness

Please note that if you are converting one or all of the above benefits, you must apply within 31 days of your termination and medical evidence will not be required.
