Conversion Options

If an employee is terminated, they may continue the following benefits at their own expense providing that they have been covered for any of these benefits listed below for at least 12 consecutive months:

  • Group Life Insurance
  • Group Long-Term Disability Injury & Illness
  • Dental Option 1
  • Health Option 1
  • Vision if applicable
  • Group Critical Illness

Please note that if an employee is converting one or all of the above benefits, they must apply within 31 days of their termination and medical evidence will not be required.
